Well made, compact score sheet - this is the one I use all the time. Gives an opportunity to record all of the information that comes in, in much the same way as a logic puzzle.
I don't understand why others are trying to recreate the wheel, unless they are merely trying to simplify the recording process -- in which case I say 'good luck'.
