We found it annoying to get the setup of On The Brink correct when playing with the challenges, since you have to switch between the base game's instructions and several places in the OTB manual. These are the original setup steps for Pandemic modified with the changes in On The Brink. The steps that are specific to particular challenges are colour-coded.
