Most of the dual expansion cards are of a big box expansion and another expansion, and are Encounters and Gate cards. Of the dual expansion Encounter and Gate cards, only a few of them refer to components from another expansion. Most of them are *thematically* related to the other expansion. Or you could have a Gate card with its two Other Worlds on two different boards.
The only dual-expansion cards which *must* be used with another expansion are the MH Blight cards, some of which refer to a component from another expansion set. However, when I've played KiY with boardgamers, we almost never draw from the Blight deck, anyway!
Here's a tip: Store the dual-expansion cards in their big boxes in penny card sleeves. They'll fit fine in the large well of the insert, and reduce the number of cards you'll put in the card-sized wells. When you do play both expansions, place their dual-expansion cards *on the top of their decks* so you get to use them!
Speaking of which, several of the new MH skills are expansion-specific, allowing the skill owner to access the goodies of the expansion at the start of the game (eg. a Cult membership or an Egyptian item). When you play with their expansion, *deal out this skill to one of the players* preferably the crazy guy who's gonna go after them regardless of what's happening in the game.
